Before we meet

Before our first scheduled consultation, I ask that you fill out and email me a new client intake form, a food journal, and client agreement form.  These can be downloaded here:
These are PDF documents that you can fill out on your computer, save and send me.
I will also invite you to fill out an online nutritional questionnaire, once you have provided your email address. 

The forms should take you about 20-30 minutes to fill out.  The nutritional evaluation asks very detailed questions but is mostly multiple choice. We will review it together for our initial evaluation. 

Before your first consultation with me, you will be asked to fill out the electronically fillable and submittable New Client Intake Form, a three day food journal, take an online nutritional evaluation, and sign the electronically submittable client agreement form.


Although I am not a doctor, if you are comfortable sharing this information, please also submit any blood or urine lab work to me at

Please get materials to me at least 24 hours before our appointment so I can prepare for our consultation.  These are required, in order for me to begin to serve you to the very best of my ability.


I will work with you to conduct several health assessments using one or more of the following tools (if you have recently completed such assessments we may use those  results).  


Blood Lab Test (BLT)

A blood test – sometimes referred to as a blood panel – is a laboratory examination of a blood sample used to check for a variety of things, including the functioning of certain organs (such as the liver, kidneys, thyroid and heart), infections and certain genetic disorders, as well as to assess an individual’s general health. 


I will work with you to get tests scheduled via Blood Lab Testing System, a low-cost service that will make arrangements for the testing to be done at a LabCorp facility near you.


Organic Acids Test (OAT)

The Organic Acids Test offers a comprehensive metabolic snapshot of a patient’s overall health with over 70 markers. It provides an accurate evaluation of intestinal yeast and bacteria.


Many people with chronic illnesses and neurological disorders often excrete several abnormal organic acids in their urine. More details about the test are on the website of the testing service, Great Plains Laboratory


Hair Tissue Mineral Analysis Lab Test with Recommendations

Mineral deficiencies and toxicities affect everyone. And these days, with our toxic exposure and toxic overloads from heavy metals, glysophate, and other environmental insults, HTMA is an and outstanding screening tool that provides key indicators behind a wide range of conditions affected by mineral imbalances and deficiencies.

Minerals are the spark plugs of life.


Without spark plugs, our bodies won’t run. Heavy metals and toxins oftentimes displace or chelate our nutrient minerals.  When our minerals are out of balance, basic cellular functions are compromised and all the other healthy things that we do, won’t stick. Hair mineral analysis lets us look at key minerals and toxic metals in a non-invasive and affordable way.

The Omega Test

Omega imbalances and deficiencies are epidemic in our society. If in our initial consultation or through a hair analysis, we discover that that there is a good deal of inflammation to address then I will recommend that you take this affordable $79 test.


Anyone with chronic inflammatory diseases, arthritis of any kind, AlZ, PD, or cancer will be asked to take this test as well. And I have an amazing Omega/melatonin product (they work better together) to fix these issues.  


Great Plains Laboratory Mycotoxin Test

The Mycotoxin Test pairs perfectly with the OAT test. MycoTOX screens for eleven different mycotoxins, from 40 species of mold, in one urine sample. MycoTOX is the most comprehensive and competitively priced mycotoxin test available.

Mycotoxins are some of the most prevalent toxins in the environment.  Mycotoxins are metabolites produced by fungi like mold, which can infest buildings, vehicles, and foodstuffs.   A majority of mycotoxin exposures are through food ingestion or airborne exposure.  In the European Union, 20% of all grains harvested have been found to be contaminated with mycotoxins. 


While I can order these labs for you, you may wish to order them through your doctor if your insurance will pay for these labs, so that you will not incur out of pocket expenses.


Additional Tests we may decide to use

Depending on your specific situation, we may decide to conduct some other types of assessments.  Some other services I use include the following: Food Inflammation, or FIT test for sensitivities to certain foods from KBMO Diagnostics, iodine testing from Hakala Research, and blood tests from OmegaQuant, to name a few.

Sessions-What to Expect

The initial consultation covering your health history and nutritional evaluation generally takes about two or more hours. So plan for that.

For clients with digestive issues, I typically recommend that you start with the practitioner-guided Revitalize Digestion 4R program:

  1. removing the offending foods in your diet

  2. replacing with good foods,

  3. reinoculating with probiotics, and

  4. repairing the gut and gut lining with a few other supplements

At the end of our session you will receive my initial dietary, lifestyle, and supplemental recommendations, along with a powerpoint presentation of your case history.  At this time we will also schedule your first follow-up consultation.

Follow up emails, phone calls, and texts are encouraged to help you get off to a fantastic start in your first few weeks or first month.  


Email communication is preferred, so please send questions that can be answered in 5 minutes or less to  I will respond generally the same day, or within 48 hours. For questions too complex for email, we will plan an office, phone or Go To Meeting appointment.

Fees & Policies

At our initial consultation, we will review your blood work if you have included it, your health history, your primary concerns and goals. At the end of our first meeting, will make initial recommendations.  I strongly recommend all clients do a hair tissue and mineral analysis HTMA Lab Test at the time of the initial consultation. It will take two weeks to get your HTMA lab results back. At this time we will also schedule our second appointment to go over your HTMA lab results. The combined cost of the two services is $450. Separately, an HTMA Lab Test is $250, so I strongly encourage you to do them both at the same time.  

Follow-up consultations are billed at $95 per 60-minute session (if the meeting goes over 75 minutes, will be prorated $45 per additional half hour). 

Scheduled appointments that are missed or not canceled with at least 24 hours notice will be charged at my regular hourly rate of $95 per hour.  Also, if you are more than 20 minutes late, the appointment will be rescheduled for a later date and the full price of the session will be charged.  Emergency situations are exceptions to this policy.
